The Coder shares responsibility for maintenance and care of all medical records. The Coder works under general supervision to assign diagnostic and procedural codes to patient charts using the hospital designated coding classification system. Coder must be able to work independently and be a confident communicator who forges a positive work relationship with physicians and staff. The candidate reviews medical records for the determination and accurate assignment of all documented diagnoses and procedures; reviews status of patients (admitted and discharged) and identifies all billable services requiring professional fee billing; codes all documented services and submit for billing; ensures coded services, provider charges and medical record documentation meet appropriate guidelines or standards; utilizes appropriate methods to ensure all documented professional services are submitted in a timely manner; maintains production standards; communicates documentation improvement opportunities and coding issues (discrepancies) to the appropriate personnel for follow up and resolution; codes any and all types of patient records; and releases information to individuals or agencies in accordance with hospital regulations and statutes of privileged communication. Performs miscellaneous duties as assigned.
Associate degree is required. Successful completion of a coding certificate program with AHIMA approval status is strongly preferred (RHIA, RHIT, CCS, or CCSP certification)
Prefer two (2) years of experience as a hospital coder or equivalent medical setting. The candidate must be knowledgeable of federal and state regulations as it pertains to coding and reimbursement. Requirements will include successful completion of orientation checklist and skills/credentials/competency requirements of specific unit.
The position requires good customer service skills. The candidate must be able to communicate effectively and demonstrate service excellence through appropriate response and use of constructive problem solving techniques when confronted with different people or situations; use of effective communication skills which foster positive outcomes. Able to maintain a high awareness of patient satisfaction and team work. Able to pay attention to detail and work in a stressful environment. The candidate must be willing to work with minimal supervision. Must be able to function independently, have flexibility, personal integrity, and be able to work effectively with resident physicians, hospital personnel, and support agencies.
Typical Physical Demands:
The position requires full range of motion including handling and lifting of boxes and/or equipment, manual and finger dexterity and eye hand coordination. Must be able to stand, walk, squat or bend and move about freely a considerable amount of time and be able to routinely lift and or move up to 10 pounds, frequently lift and/or move up to 25 pounds.
Requires corrected vision and hearing to normal range. The position requires the ability to work under stressful conditions and/or irregular hours.
