Community Care Worker I determine and re-determine need and eligibility for In Home and Family Support program. Authorizes and monitors IHFS services.

Essential Job Functions:
Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned. Determines needs and eligibility for In Home Family Support Program. Work involves conducting home visits, interviewing individuals who are older or have a disability, documenting information, determining need for services and developing service plans; administering special grant benefits, verifying data, explaining program benefits and requirements, referring individuals for appropriate services, and assuring authorizations are registered in automated systems. Authorizes and monitors CCAD services to determine if services are meeting the individual’s needs. Documents case records using automated equipment to form a record for each individual.

Knowledge Skills Abilities:
Knowledge of Gerontology, the field of disability, the dynamics of the aging process, and the special problems of the aged and disabled. Experience communicating effectively with all types of people in all types of situations. Ability to operate computer equipment. Knowledge of business English. Education which demonstrates the possession of knowledge, skills, and abilities for job performance.

Registration or Licensure Requirements:

Initial Selection Criteria:
Long Term Care/Aged and Disabled casework experience preferred. Education and/or experience that demonstrate possession of the knowledge, skills and abilities necessary for job performance. Professional experience in interviewing; counseling; in gathering of information; in programs which require applying complex policies to determine eligibility; interpreting policies to the public, clients or others. Working knowledge of automation programs including Word, Excel, Outlook and Power Point. Must be willing to travel 65-70% of the time.

Additional Information:
Occasional overnight travel stay required. Person selected will be responsible for a caseload, which may cover several counties. This position is included in a career track series and may be filled at any level within the career track, dependent upon the selected applicant’s qualifications as they relate to career track promotion eligibility requirements. If a former CCAD worker is selected as a worker within two years after holding that position, the applicant will be rehired at the classification level at which they left. DADS’ salary provisions are used to determine the hiring salary.
