Care Worker I determines/re-determines need and eligibility for Community Care Services, authorizes and monitors Community Care Services provided by contract agencies. Work involves: interviewing elderly clients and clients with disabilities; documenting, explaining program benefits and requirements; referring clients for appropriate services; and assuring authorizations are registered on automation systems. Authorizes CCAD services and monitors to determine if services are meeting the consumer's needs.
Essential Job Functions:
-Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned.
-Interviews applicants, consumers, and resource persons and conducts home/site visits to assess the need for services.
-Works effectively with consumers, their families, provider agencies and other state agency staff to compile information and ensure services are authorized appropriately and accurately.
-Conducts reviews to determine and re-determine eligibility and functional needs.
-Obtains, verifies and calculates income and resources to determine client financial eligibility.
-Documents case records using automated equipment to establish a record for each consumer.
-Develops/coordinates/explains client service and payment plans and authorizes services to meet client's individual needs.
-Serves as a consumer advocate and agency representative in locating available resources and making referrals to community resources when appropriate.
-Prepares basic level ongoing or special narrative or statistical reports.
-Monitors services provided by contract agencies to measure the effect of services delivered and to detect any problems.
Knowledge Skills Abilities:
-Knowledge of gerontology, the dynamics of the aging process and special problems of long term care service clients.
-Knowledge of interviewing techniques to obtain personal information; make inquiries, and resolve conflicting statements.
-Knowledge of the English language such as vocabulary, grammar and organization of expression for composing correspondence and reporting casework.
-Ability to establish and maintain effective relationships with consumers, co-workers, contract agency staff and staff from other federal or state agencies.
-Ability to listen to clients, exploring and reflection feelings, and present relevant alternatives describing feasibility and consequences of choices.
-Ability to appraise living conditions including physical and emotional environment.
-Ability to appraise nature and seriousness of threat to an elderly or disabled person.
-Ability to establish priorities according to relative importance and deadlines, set goals, and manage time efficiently.
-Ability to communicate orally and in writing.
-Ability to establish rapport with clients.
-Ability to communicate with community members to request assistance for clients, promotes support for the agency, or coordinates resources and programs.
-Ability to operate computer and general office equipment.
Registration or Licensure Requirements:
Current Valid Driver's License

Initial Selection Criteria:
-Prefer education and/or experience indicating the individual possesses the knowledge, skills and abilities necessary for acceptable job performance.
-Must have exceptional organizational skills.
-Prefer typing of at least 45 wpm.
-Must be willing to travel at least 80%
-Experience using e-mail and Microsoft Word processing applications and Excel.
-Experience in interviewing, dealing with Aged and Disabled individuals, decision making, composing correspondence and reporting/recording casework.
-Experience in communicating effectively with all types of people in all types of situations.
-Ability to operate computer equipment.
This position is included in a career track series and may be filled at any level within the career track, dependent on the selected applicant's qualifications as they relate to career track promotion eligibility requirements.
-Must be able to attend training which may require overnight stay.
-Position located in San Antonio: 1067 Bandera Rd, San Antonio, TX 78228.
-Position does not currently allow for compressed/alternative work schedule.
-At the point of selection for an interview the most recent performance evaluation will be requested.
-Assigned work area could be within 100 mile radius from headquarters.
-If a former CCAD worker is selected as a worker; or former CCAD unit supervisor is selected as a supervisor within two years after holding that position, the applicant will be rehired at the classification level at which they left. DADS’ salary provisions are used to determine the hiring salary.
