Facilitates the growth and community interaction of an individual with neurological impairments, learning disabilities, or autism. Provides a variety of services necessary to enable the individual to live successfully in the community while providing relief to individuals who provide primary care and support. Works one-on-one in the consumer's home and community to address a variety of life skills to include social, recreational and other activities of daily living.

ALBANY- We are looking for an experienced direct support professional
to work one on one with a 21 year old man who lives with his mother in
Albany. Hours would be Tuesday 1:00-5:00 minimally with flexibility
for more time as needed throughout the week. He wants support to
maintain and expand his current community inclusion activities,
including swimming, participating on the Special Olympics track team,
volunteering, walking, food shopping, and continuing to develop
effective communication skills
ALBANY- We are looking for a DSP for Community Habilitation to work
with a 27 year old woman who lives with her mother in Albany. We are
looking for someone to support her approximately 10 hours per week,
2 to 3 half days (either mornings or afternoons), Monday –Friday.
Skill development includes organization, planning, communication, and
safety. She is quite social and eager to learn.
DELMAR- A 16 year old living with his parents and twin sister would
like a DSP for Community Habilitation on Wednesday afternoons. He is
looking for assistance with daily living skills to learn Spanish more
fluently, refine his money skills, work on his computer completing
spreadsheets and creating apps, and to access his community for
dining, shopping, etc. He enjoys history and technology.
COHOES- A 17 year old boy living with his parents and younger sister
would like to access the community for comm hab activities such as
basketball, football, shopping, dining. He is working on remaining
safe and appropriate in all environments. He would like assistance
with problem solving to make informed choices and safe decisions.

Columbia County Opening:
KINDERHOOK- Part time Community Habilitation DSP needed to support a
25 year old woman who lives with her family in Kinderhook. We are
looking for someone to work weekend days, approximately 3-4 hours each
session, in both her home and her community. Supports needed include
encouraging her to use words to communicate, hands on assistance with
personal hygiene, simple meal preparation, and being safe and
appropriate in the community. She enjoys feeding horses, going for
walks, shopping, going to church, movies and out to dinner.

Saratoga County Openings:
CLIFTON PARK- An older teen boy living with his family would like an
additional worker on M or W after 3:00 to assist him with
independence, personal hygiene tasks, and safe and appropriate conduct
in the community. He enjoys going to the Y, simple cooking, dining
out, going to the library, sports and playgrounds.
CLIFTON PARK- A 26 year old man with a developmental disability who
lives in Clifton Park is looking for support 4 hours each on Sunday.
He lives in his own apartment and is quite independent and capable.
He seeks support for social and recreational opportunities, connecting
with others in his community, expanding his interests, and exploring
volunteer opportunities.
MECHANICVILLE- A male in his 30’s living with his parents is looking
for a community habilitation worker to access the community on the
weekends. He would like assistance with appropriate and effective
communication and safety in his community. He enjoys being a part of
his community and getting to know people he sees regularly.
MALTA- We are looking for a DSP to work 2 times each week, for 3
hours each session with an 8 year old boy with a developmental
disability who lives with his parents in Malta. He is very active,
loves swimming at the Y, walks, bowling, and the library. He requires
significant support in areas of self care, following directions, and
safety, both at home and in his community.
Schenectady County Openings-
Guilderland Area Schenectady- A male in his 30’s, living with his
family, is looking to go out a few afternoons each weekday from about
11:00-2:00. He would like support with exercising, healthy dining,
shopping, walking. He would like assistance with decision making,
assertiveness, increased independence and money skills.

Qualifications

High school diploma or general education degree (GED). Medication Administration based on program need, First-Aid and CPR certification or eligibility for certification. SCIP/PROMOTE certification based on program need. Daily access to an automobile is required. A valid New York State Driver's License and ability to drive and driving record acceptable for agency insurance coverage are required. CPR and First Aid certification or eligibility for certification. Applicants hired after April 1, 2005 must be cleared and maintain acceptable record under the NYS OPWDD mandated criminal background check process.
