This position is grant funded for one-year. The Community Health Representative works mainly offsite performing a wide range of duties which includes recruitment of diabetics for Southside/Dodson Avenue CHCS' Diabetic Project, community outreach, some home visitations and referrals to health centers' services. Serves as a liaison and resource person for the health centers'. Must have computer competency, excellent customer relations, and organizational skills. Position will require walking, driving and mostly evening and Saturday work.
High school graduate with additional college education preferred, basic computer skills.
One to three years of recent local community related experience. Knowledge of local social programs, grass roots community leaders and ability to work with diverse groups.

Department Position Summary:
The Community Representative will primarily recruit and assist diabetic clients in completing a Diabetes Survey for the Diabetes Project. Person will serve as a liaison and resource person who connects potential patients to health centers' services. Also will assist the Diabetes Project nurse in following up on health centers' diabetic patients who have failed their appointment(s).
