Nemours is seeking a Community Health Worker to join our team in Delaware.

To keep pace with the changing health care environment and build a healthier future for our children, Nemours has embarked on a multi-phase expansion to create a children’s hospital like no other in the region. Upon completion in June 2014, the hospital will include private, family-centered, technologically advanced patient rooms, a new Pediatric Intensive Care Unit and a new expanded Emergency Department.

The primary role of the Community Health Worker will be to provide support for low-income families with children with asthma in Dover, Delaware. Making use of close personal ties to the targeted community and a background similar to the families to be served, this position is responsible for: advocating, facilitating and organizing access to health, community and social services. The position will also work with families to ensure that they understand and can follow instructions on the medical management of their child’s asthma. The Community Health Worker will serve as a member of a large grant team focused on improving asthma outcomes for children in three neighborhoods. We are currently recruiting for three Community Health Workers to be based out of our Dover practice location.

Responsibilities

Provide case management of non-medical needs to a small caseload of high-needs families with at least one child with asthma.

Conduct home visits and work with others to promote healthy, asthma trigger-free households.

Collaborate with the primary care physician, members of primary care team, and evaluation staff to track and document non-medical issues that affect health.

Provide in-office and home-based coaching to families on asthma management.

Maintain regular and ongoing communication with participating families, assisting families in developing skills and obtaining supports to promote their children’s health.

Provide both on-site and phone support to participating practices.

In collaboration with other project staff, identify, create and nurture relationships with local agencies, schools, churches and other programs.

Design and facilitate roundtable discussions for a broad variety of stakeholders.

Additional Responsibilities

Provides input to all team members on how to best serve families of children with asthma.

Collects baseline and follow-up data from families.

Recognizes situations appropriate for referrals to various social service agencies and programs.

Assists stakeholders in identifying and pursuing personal and community goals.

Identifies and works with advocacy groups.

Speaks publicly about the project and children’s health issues.

Requirements

One year of previous experience is required.

A high school diploma is required; some college preferred.

Travel throughout the Dover area is required; applicant must have a valid Delaware driver's license.
