Supports the heart health add-on to West Side’s breast and cervical cancer screening program.
Will facilitate blood screenings for low-income, underinsured and uninsured women for cardiovascular disease risk factors.
Provides a structured lifestyle intervention to help them reduce their risk by becoming more physically active, make healthier food choices, and stop smoking.
Specific Duties Include:
Communicate effectively and cross culturally to educate patients on health issues, increase knowledge of preventative health care, and increase utilization of available health care resources.
Organize and conduct health education/outreach activities and screenings.
Assist with other duties such as arranging follow-up medical appointments and transportation.
Community Health Worker certification preferred.
Minimum one year of outreach, case management, health education, or clinical experience.
Knowledge and familiarity of health promotion and health education in a health care system.
Bilingual English/Spanish strongly preferred.
