Assist Community Integration Specialist in providing Habilitation and Home Group (for individuals assigned) services to individuals in Day Habilitation, in providing of consultation and the Habilitation services or other Individualized Service Plan components for which responsible, in assuring compliance with all applicable Day Habilitation Standards. Will work as a team member with Community Integration Specialist and case manager to ensure that Day Habilitation components of the ISP are delivered. It is the responsibility of the team to ensure that all services are individualized and based on choice.
Requires an Associate's Degree in a human service field or a high school degree and one year of experience with the developmentally disabled are preferred. Is required to meet LAI's Vehicle Operator Requirements. Subject to Background checks including; Fingerprinting, New York State Child Abuse Registry, and Driver's license (if position requires driving). Physical ability to lift up to 50 lbs. Must be able to succcessfully complete all required trainings including but not limited to First Aid, CPR, Medication Administration and SCIP (Strategies for Crisis Intervention Prevention).
