This position works in collaboration with the CSS participant and their Circle of Support, promotes the realization of specific outcomes while adhering to the principals of self-determination and person centeredness. A strong emphasis is placed on creating meaningful community connections.

Essential Duties and Responsibilities include the following. Other duties may be assigned. §
Provides instruction and assistance to individuals in areas of daily living including but not limited to personal care, cooking, home maintenance, social connections, travel training, constructive use of leisure time and personal finances. §
Participates in a person centered approach to program plan development and participates in activities to build competencies and integrate individuals into their communities. §
Embraces an approach of self-determination. Works for and with the individuals on their own individualized outcomes.

Qualifications
High school diploma or general education degree (GED). A valid NYS driver's license and ability to drive and driving record acceptable for agency insurance coverage are required. Must be cleared and maintain acceptable record under the NYS OPWDD mandated criminal background check process.
