Position summaryThe Community Liaison is responsible for the implementation and coordination of efforts to educate members of the community to the services offered by Home Hospital Home Health Care (including infusion Pharmacy services) and St. Elizabeth Hospice. In addition, the coordinator maintains liaison with other community health organizations and reports back to the Regional Director on unmet community needs for consideration in the development of new services. Works with the entire management staff to provide feedback on community perception of agency services. Develops and implements marketing plans. Reports to the Regional Director, Home Health and Hospice.Essential job functionsCommunity RelationsImplements systematic approach to community education process including regular contact with members of the community with an emphasis on home health and hospice services. Serves as a liaison with extended care facilities as well as other facilities to facilitate the transition to home health and/or hospice services as appropriate. MarketingDevelops and implements the Home Health and Hospice Marketing Plan. Integrates marketing activities with the Greater Lafayette Health Services, Inc. Public Relations Department. Maintains liaison with tertiary care centers in major metropolitan areas so that residents returning to the local area have access to local home care and hospice services. Utilizes referral, admission and market statistics to identify target markets. Assists in the identification of clinical programs/protocols and related marketing materials that are tailored to specific referral sources and their patients' needs. Monitors the effectiveness of marketing efforts/activities. Monitors referral source satisfaction. Other ResponsibilitiesAssists with Performance Improvement. Responsible for timely submission of all data. Identifies community needs and reports them back to the agency. Participates in the development of new programs to meet community needs. Functions within the parameters of all applicable state and federal regulations. Knowledge, Skills AbilitiesAbility to operate current software packages with department. Basic knowledge of marketing principles and sales strategies. Sensitive to issues of loss and grief. Graduate of an accredited school of nursing or comparable degree in an Allied Health field preferred. College degree preferred. Minimum of one year successful community resource experience preferred. Home Health or Hospice experience preferred. Minimum of three yeas experience in healthcare or related field preferred.Minimum qualifications EducationGraduate of an accredited school of nursing or comparable degree in an Allied Health field preferred.
