Under the supervision of the Director of Community Living, the CLM will be responsible for providing overall comprehensive care to assure that the people supported, thrive through coordination of medical, social, financial, religious, recreations, work, housing, employment or day services. The CLM will assure that staff supports are in place for the person supported to live as full participants in their home and community utilizing a person centered plan to develop, implement and monitor the Individual Support Plan (ISP) while focusing on the environment, health, safety and well being of the person supported. The CLM will also supervise Direct Support Professionals and assure that the residential program is delivered in an appropriate, caring, professional, and respectful manner; will coordinate Case Management; will provide individualized home care, and coordinate community resources and supports.
High School Diploma or Equivalence required. Four year degree from an accredited college a plus. Must have three to five years management experience in a Residential setting working with people with development disabilities.
