SPECIALIST
The Chautauqua County Health Network (CCHN) has an immediate opening for a grant funded Consumer Engagement Specialist to support a variety of community health and clinical initiatives focused on strengthening patient self management skills. The incumbent will support health care providers throughout the Chautauqua region to link their patients with supportive community services to improve health outcomes. Working in a team environment, primary responsibilities will include: resource identification;community agency and provider outreach;referral process development; chronic disease program education; patient empowerment training; event coordination; data collection and entry; timeline administration; program material development and dissemination; and partnership development.
The ideal candidate must be a self starter who is organized and can manage multiple priorities, be know- ledgeable about local programs and health service resources, possess excellent verbal and written communication skills, be proficient in the use of Microsoft Office programs, and be willing to work evenings and weekends as needed. Experience working with grants, leading meetings, and facilitating programs is a plus. Reliable transportation and valid NYS driver's license a MUST as travel throughout WNY will be required. Bachelor's degree in health related field preferred.
