Under general supervision of the Director of Care Coordination, the Continuing Care Financial Assistant will perform non-professional and clerical aspects of the discharge planning process. This role will provide a prompt, systematic, reliable, and efficient supportive presence to the case managers and the unit’s licensed nursing staff in their continuing care activities in order to enhance the delivery of these services to patients. In addition this position requires special skills that include knowledge of government program and private insurance requirements, and financial training. Responsibilities include providing financial assistance and counseling for patients with an inability to pay for services. This individual must maintain ongoing communication with case management and the nursing staff, be able to take direction and to assess data reflective of the patient’s status and their financial needs and interpret the appropriate information to assist with the continuing care arrangements.
High school diploma or equivalent, Associates degree preferred
Basic skills in math, reading and writing English required
Experience with a federally funded finance program
