Our client, an expanding independently owned LTC pharmacy in the St. Louis. MO. area is seeking a CONSULTANT PHARMACIST on a long-term contract basis.
Initially the individual in this role will consult with one home in the St. Louis area ( approximately 6-8 hours monthly) with the potential for more homes as business continues to expand. Previous LTC experience as a consultant pharmacist is required. The consultant pharmacist will provide direction and oversight on all aspects of the acquisition, disposition, handling, storage and administration of drugs in a LTC facility with approximately 120 patients. Duties will include:
Monthly (or as needed) medication regimen review (MRR) including recommendations and reports for the facility and pharmacy
Quarterly pharmacy QA report and attend QA and process improvement meetings
Disease State Management
Medication cart and medication room evaluations
Development of clinical materials for facilities and physicians
Facility survey preparation scheduled and “on-demand"
Additional committee meeting attendance as requested
Review of pharmacy related policies and procedures as requested
Communication with pharmacy, prescribers, resident's families, etc on behalf of facility regarding medication concerns
Facility staff education/inservicing Must have a valid pharmacist license issued by the Missouri State Board of Pharmacy, a BS. In Pharmacy or Pharm D degree and a minimum of 3-4 years experience as a consultant pharmacist in a long-term care setting. Also required are excellent communication skills, both oral and written, and the ability to work independently.
