Bachelor's degree in social work, gerontology or health related field or equivalent work experience.
LVN or BA with previous experience with care coordination and/or the geriatric population.
1-3 years previous clinical experience working in a medical or social service setting with a geriatric population.
Knowledge of available health care and relevant community resources appropriate for the Medicare population.
Knowledge of individual and family development over the life span, and the influences of cultural and spiritual values in health care.
General knowledge of illnesses, current treatments, and their physical and psychosocial effects on the individual.
Knowledge of applicable laws and regulations, government and insurance benefits, or where to find this information when needed.
