High School graduate

Second language a plus

Knowledge of medical terminology

2-4 Years previous healthcare scheduling experience

Ability to demonstrate Advocate s Mission, Values and Philosophy while creating an environment that supports and sustains effective communication with clinical staff, therapy vendors and most importantly the homecare patient and/or family

Exceptional interpersonal skills-able to work well with diverse personality types in a fast paced environment internal and external customers.

Ability to coordinate and perform multiple tasks both effectively and efficiently

Able to work under deadlines and time constraints

Computer /data entry experience

Working knowledge of Microsoft Word, Excel and Access as well as the ability to function in multiple mainframe database applications.

Detail oriented N/A Weekend hours - Friday, Saturday and Sunday.

Job Description:
The purpose of this position is to help achieve the Vision of Advocate Home Health Services by performing the duties of this position in a cost effective, results orientated and customer focused manner. This position is a critical component of the operating unit and as such meeting individual performance expectations and contributions to team performance goals is required.

This associate must be able to work weekends, work 1summer and 1winter holiday and have experience scheduling therapy and nursing visits for clients. Home health scheduling experience is recommended for this position. This is a PART TIME position.

The primary responsibility of the Weekend Scheduling Coordinator is to coordinate and schedule nursing and therapy services along with other interdepartmental services required for home care at the patients residences. These interdepartmental services include the home care team, multiple therapy vendors as well as the homecare patient. Responsibilities include: assigning appropriate skilled nursing and therapy staff to patients, monitoring productivity of nursing staff and adjusting schedules as appropriate, making cost-effective decisions regarding scheduling of contracted therapy services.

Accountabilities:
1. Nursing/Therapy Coordination - Serve as internal point person and processing agent for all new home care referrals as well as serving as the Advocate representative responsible for coordinating ordered disciplines with staff therapists and contracted therapy agencies in the Home Care Region.

2. Coordinate scheduling of home health aides, therapy disciplines, and nursing to ensure appropriate and timely patient care

3. Performs other support responsibilities as requested.

Experience with scheduling clients with RNs, CNAs, and therapists is required for this position. This associate will work WEEKEND program. PART TIME will also be considered for this position.
