The Spiritual Care Coordinator is responsible for providing spiritual counseling and support to patients and families, within the spiritual/religious frame of reference for each individual. The Spiritual Care Coordinator, through a collaborative process, is responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating the strategies and services required to meet a patient/family’s end-of-life spiritual needs. Relationship-centered care principles and holistic philosophy will be utilized to create an outcome-based plan of care which will be continually adjusted to meet changing needs of the patient and caregivers and help facilitate a pain-free, peaceful, sacred death.
Master’s Degree in counseling, theology, or related field or equivalent experience required. Previous experience in spiritual and/or bereavement counseling required. Evidences understanding of own personal beliefs while able to journey with and focus on others’ needs, experiences and beliefs. Must demonstrate understanding and commitment to end of life care. Computer Literacy required. Valid Driver’s License required.
