Our Director of Care Management is critical to the success of St. Joseph's Medical Center and require the full understanding and active participation in fulfilling the Mission of Dignity Health. It is expected that our employees demonstrate behavior consistent with the Core Values.

The Director of Care Management is responsible for five areas: Care Management, Discharge Planning, Clinical Documentation Improvement Program, Social Services, and the Clinical side of our RAC program. The Director is responsible for the overall direction and management of these areas, including planning, organizing and directing all activities, staffing, performance improvement/efficient use of clinical resource, and reporting within the Medical Center as well as external (i.e. government and regulatory) and within System. The Director has additional oversight over the Physician Advisor, who provides medical guidance on Care Management issues. The Director guides planning and organization according to the needs, requirements, and policies of the Medical Center, the affiliated medical groups and health plans, Dignity Health, any Federal and State agencies, and according to standard practices of the professions
under the director's accountability. The Director is given general guidance and direction by the appropriate administrative head (currently CFO) and is expected to function with initiative and independent judgment. The Director participates regularly in Medical Center, Service Area, or Corporate meetings pertinent to the accountable areas, and also participates in corporate strategic planning and performance improvement teams and programs as necessary. The director regularly interfaces with medical staff members, various medical staff committees, and presents and manages initiatives as appropriate or assigned. The director works across and with multiple department with the Medical Center and within the Service Area.

This position requires the full understanding and active participation in fulfilling the Mission of the Organization. It is expected the director will demonstrate behavior consistent with the Core Values of the organization. The director will support the Organization's strategic plan and the goals and direction of the Performance Improvement Plan (PIP).

Comprehensive knowledge of care management and discharge planning
Medicare regulations, RAC appeal process,Medi-Cal requirements, and generalcommercial admission requirements.
Working knowledge of Interqual criteria and use.
Five or more years progressiveSupervisory and management experience in a mid-size to large hospital setting.
Effective communication skills, especially with medical staff
BSN degree or equivalent
Current California RN License

Working knowledge of finance and budgetary process, and government billing regulations.
Knowledge of Midas, and Meditech.
Knowledge of Conditions of Participation and Joint Commission standards.
Master's prepared RN
