The Director of Operations will serve as the overall project manager for the Continuum Health Home Network (CHHN). Working with the CHHN Clinical lead, the Director of Operations will assume responsibility for day-to-day functioning of the CHHN, including: receiving and distributing health home member names, tracking care managers assigned to members, ensuring that QI targets are being met and that all CHHN staff are adhering to NYS DOH State Plan Amendment guidelines for Medicaid Health Homes implementation.

To qulaify for this position you must have: a minimum of 5 -7 years of clinical quality improvement and project management expereince of which 2-5 years of experience was as a quality improvement advisor in hospital and outpatient environments, and 2-5 years in direct supervision of project team(s), with success managing at least one large-scale project to completion.In addition the qualified candiate must have expereince in Qu ality Improvement , Change Management and Conflict Management methodologies; Specific knowledge of change concepts, including Chronic Care Management, and Patient-Centered Medical Home.
