Stonington Institute began serving recovering
alcoholics in 1957 as Starlight Farms, a working farm where adults in
recovery were introduced to the principles of Alcoholics Anonymous.
Today, we are the longest continually operating, free-standing,
voluntary treatment facility in New England. Our main campus is
located in North Stonington, CT, in its original private and peaceful
rural setting. We have additional clinical sites in Groton, CT.
Transportation and off-campus housing assistance make our programs
and services accessible to a statewide clientele. Those programs
include: medical detoxification, rehabilitation/residential
treatment, partial hospitalization, intensive outpatient treatment
(both day and evening) and ambulatory detoxification, as well as
residential addictions treatment for active duty and retired military
servicemembers. The Institute accepts private insurance, state-funded
plans and self pay arrangements.
The Discharge Coordinator works directly with
Clinicians to ensure that each client's discharge plan
is complete. Reviews and audits plans, identifies barriers
to care and develops referral sources, with the intention of
maximizing each client's potential for sucess beyond the
program. Develops and maintains contact with clients,
client families and other service providers in order to coordinate
the medical, clinical, social and related needs of clients during
treatment. Develops and implements discharge plans and ancillary
client services both during treatment and after discharge. Conducts
continuing stay authorization reviews with insurance and managed care
companies, as necessary and appropriate. Works closely with
the RN, Clinician, and other members of treatment
This is a part-time position with flexible hours,
Masters degree in Social Work or related field.
Discharge planning experience required, preferably with the
co-occurring adult population. Familiarity with community
resources for people with mental illness is essential.
