Lutheran Social Service of Minnesota seeks a full time, benefit-eligible Designated Coordinator (DC). The DC will be responsible for supporting 2 homes for individuals with disabilities. The position of DC includes support planning, attending medical appointments, maintaining working knowledge of individual service plans, staff supervision, and maintaining licensing requirements. As a DC you will participate as a member of the team, fostering an environment which supports individuals in pursuit of their hopes, dreams and personal outcomes. The Designated Coordinator will be responsible to oversee the program needs of the individuals and ensure quality services are delivered to the individuals we support, which may include accessibility at all times to support and maintain a safe and secure environment. Requirements: The designated coordinator must minimally have:
(1) a baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or
(2) an associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or
(3) a diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or
(4) a minimum of 50 hours of education and training related to human services and disabilities; and

(5) four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).

Successful candidates possess the ability to be flexible in many different situations and will have strong written and verbal communication skills. All applicants must have a valid driver’s license and pass a background study and motor vehicle record check.
