SUMMARY

Provides services and education to dialysis patients and authorized caregivers in clinical settings to support and maximize the patient’s nutritional well-being. Based upon nutritional assessment and renal standards of practice, develops a nutrition care plan; evaluates and documents through written records; educates the patients and/or family members; and provides on-going care for dialysis patients.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive. Other duties may be assigned.

Patient Care
·
Assess nutritional status to determine appropriate nutrition intervention(s) and recommendations
· Selects or develops education materials and methods appropriate for individual patient.
· Participate in Interdisciplinary (IDT) Plans of Care (POC) based on assessments and reassessments per CMS, company and state guidelines.
· Develop individualized Medical Nutrition Therapy including meal plan(s) for each patient
· Monitor nutritional status, laboratory values, dialysis adequacy, adherence to diet and response to nutrition related medications and therapy to determine appropriate nutrition intervention and recommendations; Instructs on appropriate renal diet in accordance with physician’s orders and clinical assessment results.
· Responds to medical staff concerns and referrals in a timely manner and refers to
appropriate outside resources.
· Revise and update nutritional recommendations and POC as needed
· Review monthly lab results with each patient and/or caregiver (per HIPAA guidelines) and provide patient with a printed nutrition report; documents any problems; and through education relates diet, medication, and dialysis to the lab results
· Function as Mineral and Bone Disorder Specialist
· Participate in anemia management
· May function as the manager of bone disease per unit specific requirements.
· May function as the manager of anemia per unit specific requirements.
· May function as the manager of adequacy per unit specific requirements.
· Functions as a continuous liaison with members of the health care team regarding the nutritional care of patients
· Participate in physician rounds, patient care conferences, QAPI meetings, facility staff meetings and in-services
· Serves on Quality Assurance Committee and other committees as needed.
· Develops, organizes, and conducts in-services in the area of nutrition to staff, community,
and/or health professionals as required and as needed.
· Maintains an interest and stays current on issues related to the field of renal nutrition. May actively participate on a local, regional, and/or national level for renal nutrition.
· Maintains continuing education requirements as outlined by the Commission on Dietetic
Registration, the credentialing agency for the American Dietetic Association, and any state licensure board.
Documentation
·
Evaluate outcomes and identify factors that may contribute to undesirable results and document appropriately
· Document nutrition progress at least monthly
· Update POC for nutrition related issues based on outliers and timeline goals and document according to Federal, State and company requirements
Communication Skills
·
Contact and collaborate with extended care facility staff, family members, or others appropriate medical personnel to facilitate adequate and appropriate nutritional care
· Effectively communicate pertinent nutrition issues with the medical team and with patients
· Present nutrition related in-service for clinical staff at least annually
Perform other duties as assigned, including, but not limited to:
·
Work hours required to provide quality patient care; work flexible schedule as needed to meet patient care needs
· Work in multiple facilities and travel to cover multiple facilities or to attend meetings as needed
· Consistent, regular, punctual attendance is an essential requirement
· Attend staff meetings, phone conferences and training as needed and required
· Know, understand and follow patient and employee guidelines, employment policies, and department, clinical or company procedures
· Assist in the training of new dieticians
· Practice within boundaries established by state law.
· Ability to lift 1 lb. to a maximum of 20 lbs. and able to hear and interact by phone and in person clearly and efficiently. Able to perform repetitive standing, sitting, stooping, walking, stretching, reaching and use full range of body motions. Ability to use computer and all peripherals as needed to meet goals.
SUPERVISORY RESPONSIBILITIES

This position has no supervisory responsibilities.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE

Bachelor’s Degree in Nutrition, Dietetics, or similar area from accredited university required
· Prefer one year of clinical nutrition experience as a Registered Dietician preferred
· Completion of approved training program as required by the company and CMS guidelines
· Basic computer skills and proficiency required in MS Outlook; proficiency in all required computer systems of the company including Pearl , etc. within 90 days of employment.
CERTIFICATES, LICENSES, REGISTRATIONS
·
Must be licensed to work as a Registered Dietician in accordance with state(s) regulations in state(s) employed.
· Registered Dietician status with the Commission on Dietetic Registration (credentialing agency) required
· Optional: CSR with the Commission on Dietetic Registration.
· Current CPR certification
