JOB SUMMARY:

On a daily basis, conducts individual nutritional risk assessments, planning, education, and monitoring of all Women’s Health Center (WHC) prenatal patients, with emphasis on nutritional requirements for high-risk OBS patients (i.e. diabetics, hypertension, overweight). In accordance with Medicaid Prenatal Care Standards, incumbent provides for appropriate patient nutrition education/counseling and documents nutritional assessment, risk status, and plan of care in patient’s record.

DUTIES AND RESPONSIBILITIES

•Interviews Women’s Health Center (WHC) patient for nutritional histories, assess nutritional care of patients using objective and subjective information relevant to patient’s ages; records information in medical record
•Provides nutrition education and counseling for each pregnant woman – regarding infant feeding choices discussed with the woman during prenatal visits and immediately postpartum
•Documents and monitors prenatal nutritional assessment, risk status, and plan of care in patient’s record
•Review medical record and makes recommendations to medical providers when necessary
•Instructs patients on prescribed diets with written order from MD, tailoring communication according to patient’s ages, physical activity history, and nutritional risk
•Conducts nutritional education and support groups for all prenatal patients in the Women’s Health Center, with emphasis on high-risk patients
•Leads nutritional care aspects of WHC Performance Improvement Program
•Conducts training of WHC staff to support their interaction with patients regarding pregnancy and nutrition
•Monitors and evaluates WHC staff’s nutritional guidance of patients on an ongoing basis
•Position would also include rotation coverage of acute care units as needed

MINIMUM REQUIRED QUALIFICATIONS:

•4-year college degree required
•Registered Dietician required
•At least one year experience as a Registered Dietician in outpatient clinic setting required, preferable relevance to Women’s Health clinic/center
•At least one year experience in inpatient care setting required
•Strong knowledge of Microsoft Office programs and applications
•Bilingual preferred (Spanish, Arabic, Creole)
