Our Community Strategies division offers residential and community-based services to adults with developmental disabilities, providing our consumers the security and support needed to reach their full potential. Families and state social services departments are important partners in creating individual programs to improve living skills and foster independence, including becoming active participants in their communities.

The Direct Support Professional (direct care) is responsible for supervising and supporting consumer activities specific to service plan goals and objectives.

  • Supervises and transports consumers during assigned hours, this may include, job search, medical appointments, weekly recreation, shopping trips, and other necessary trips.
  • Supports and teaches consumers new skills including, but not limited to, those specific to goals stated in the Individual Service Plan (Service Agreement?).
  • Provides a positive, supportive role-model for the consumer.
  • Reinforce appropriate consumer behavior through direct feedback, role-playing, and behavior modification techniques.
  • Assist consumers with self-care\activities of daily living skills as appropriate. This could include turning and rolling the individual or utilizing a track or mechanical lift to assist in dressing, bathing, etc.
  • Administer medications (orally or through an auto injector); operate a nebulizer machine and/or a suction machine.
  • Drive and operate a handicap accessible vehicle and the use of wheelchair locking mechanisms
  • Documentation and timely submission of all progress notes, weekly summaries, attendance, and all other consumer documentation deemed necessary by the company.
  • Become certified in PAC (Proactive Alternative for Change) training, maintain a current PAC certification and have the ability to utilize the PAC techniques as necessary.
  • Direct Support ProfessionalAttends all weekly, monthly, etc. meetings specific to the client.

Strong communication and interpersonal skills, a valid New Hampshire’s driver’s license, 100/300/100 automobile insurance, no findings with a NH criminal records check or client rights violation.

Minimum Qualifications AS/AA or 2 years education or experience
