In business for over 25 years, WILLCARE operates Certified/Medicare, Long Term Care, Licensed/Private Duty, as well as non-medical services and supplemental staffing agencies. We provide qualified skilled and non-skilled services focusing on preventative, rehabilitative and therapeutic care to our clients in all applicable settings. We have branch offices in Western NY, Hudson Valley NY, Connecticut, and Ohio.

Job Title: Director of Nursing

Job Location: Middletown, NY

Job Department: Administration


Responsible for the clinical direction and supervision of patient care services. This position is responsible for the development and direction of all nursing / clinical services.


Evaluate policies and procedures for effectiveness in delivery of quality and safe home health care services.

Implement corrective action for policies and procedures that are not effective in delivery of quality and safe delivery of home therapy.

Ensure that patient clinical records reflect quality and safe delivery of home care services and are within accepted standards.

Ensure nursing supervision is available at all times during Agency operating hours.

Ensure case management activities are conducted to promote efficient and effective services.

Ensure on the job supervision and performance appraisals are conducted according to Policy and Procedure.

Ensure in home performance evaluation are conducted according to Policy and Procedure.

Consult with nurses regarding professional growth and documents.

Ensure disciplinary action is instituted as needed, according to Policy and Procedure

Ensure competency evaluations are performed according to Policy and Procedure.

Prepare office and complies with surveyors from Federal, State and Local and Contractual Authorities.

Monitor and evaluate home care staff compliance with appropriate policies and procedures.

Ensure current professional Licensure and qualifications for home care staff.

Document home care agency records and patient clinical records to demonstrate compliance with Agency Policies and Procedures and Federal, State and Local rules and regulations.

Assist with the review and revision of home care policies and procedures, as necessary, to reflect compliance with Federal, State and Local rules and regulations.

Assist with the development and review of training curriculum additions and / or changes.

Provide leadership for performance improvement initiatives.

Provide administrative on call responsibilities.


Bachelor’s Degree in Nursing and two years’ experience as a supervisor in the health care field, OR Bachelor’s degree in a health related field and four years’ experience in the care field of which at least two must be in community health and at least one in year must be in a managerial or supervisory capacity, OR Associates degree in nursing, or a three year diploma in nursing; and at least four years work experience in a health care field of which two years must be in community health and two years in a supervisory role.

Currently licensed and registered to practice with the State Board of Nursing.

BLS certification strongly recommended.

Knowledge of State and Federal Home Care Regulations.

Demonstrates leadership, organization and communication expertise.

Excellent verbal and written communication skills.
