FTE: 1.0
Schedule: Generally 8 a.m. - 5 p.m. with occasional evenings and weekends to support service delivery.

POSITION PURPOSE
To serve as a member of the agency’s Leadership Team and envision, participate in the development of, oversee, and manage all Social Services, Education & Prevention Programs and Case Management as well other projects assigned under contract to a nonprofit Federally Qualified Health Center specializing in the provision of services to low-income people living with HIV/AIDS. Oversee all staff assigned to these programs and projects, to include sub-contracted agencies and individuals, to assure delivery of high quality program outcomes, to ensure all resources are being utilized efficiently and effectively and to ensure compliance with regulatory agencies.

PRINCIPAL RESPONSIBILITIES
„« In alignment with agency mission, community service values, guiding principles, and strategic plans, set goals, inspire staff, monitor work, and evaluate results to ensure that objectives to respond to client needs and fulfill operating requirements are met for 1) Social Services including housing programs, community center psychosocial activities and food assistance, 2) Education & Prevention Programs including outreach, community health education, early intervention services and HIV/STD testing, and 3) Case Management including needs assessments, referrals, benefits counseling, care planning, linkage to care efforts and eligibility screening;
„« Develop and implement systems, policies, procedures, workflows, problem solving, technologies, priorities and processes needed to enhance and improve the department’s current activities as well as its projected growth ensuring a seamless and productive interface with Clinical Services and other departments.
„« Maintain current knowledge on best practices, evidence-based models and other keys to success employed by benchmark institutions, researchers in the field and relevant government agencies.
„« Effectively assess, deploy, mentor and champion the talent and expertise of staff to achieve strategic goals.
„« Participate in the recruitment and hiring process of all department personnel; Monitor and approve all staff timesheets, schedules and leave requests; Perform evaluation of staff in a timely manner; Supervise volunteer staff assigned to department;
„« Collaborate with grants department on development of funding proposals (including program descriptions, budgets, and work plans), and reporting requirements.
„« Monitor the operational delivery of services funded by a diverse array of public and private sources with complex and competing terms and conditions and contribute to the preparation for and assist with the hosting of on-site audits. ,;
„« Ensure timely preparation of necessary data and statistical analyses for inter/intra agency distribution;
„« Ensure HIPAA compliance as well as compliance with applicable rules, regulations, laws, standards of care and contract terms enforced by agencies such as the State Office of AIDS, HRSA, CDC, etc.
„« Build trusting relationships with clients when opportunities arise that model exceptional professionalism; Expeditiously address client grievances and complaints in accordance with agency policies;
„« Evaluate new potential referral services in the community; maintain a positive working relationship with local NPO’s, ASO’s and social services providers to explore and enhance D.A.P.’s role in the safety net of community services;
„« Review trends in expenses, outputs and outcomes monthly, making notations of over and under budget spending to the CFO,
„« Review, approve, and ensure the timely submission of invoices, check requests, staff salary allocations, etc.;
„« Responsibility for departmental quality improvement/quality assurance, development and monitoring of measureable outputs and outcomes, and integration of learning to celebrate areas of excellence and apply course correction as needed. Assist Program Managers with measuring service delivery impact and individual performance.
„« Prepare monthly update reports for the Board of Directors;
„« Collaborate as requested to support accurate and up-to-date messaging delivered by colleagues including the Departments of Development and Marketing & Communications.
„« Ensure staff are linked to, and engage in, trainings as appropriate to meet professional development needs and any grant guidelines;
„« All other relevant duties as assigned.

TRAINING, EXPERIENCE, ABILITIES
„« Minimum 7 years’ experience with progressive responsibility in the delivery of client services in a grant-funded social service nonprofit organization or public health setting;
„« Bachelor’s degree required;, Master’s degree in relevant field preferred. (Additional qualifying experience may be considered in place of academic credentials);
„« At least two years’ experience in a supervisory role with budgeting responsibilities;
„« Knowledge of evidence-based interventions, familiarity with effective models and sensitivity to issues that inform a successful social service delivery model meeting the needs of populations affected by poverty, poor access to health care, stigma and discrimination, unemployment, housing and food insecurity, chronic conditions such as HIV/AIDS, and mental illness and chemical dependency, .
„« Knowledge of program planning, implementation and evaluation; budget preparation and control; grants compliance; principles and practices of supervision and training;
„« Familiarity with HIPAA rules and regulations;
„« Bilingual Spanish preferred.

„« Ability to operate standard office equipment such as personal computer, fax, calculator, copy machine, scanner, etc. as well as Microsoft Office applications including Excel and Outlook; Familiarity and comfort level navigating electronic health records.
„« Motivate and lead staff to excel and grow professionally;
„« Ability to understand, implement and comply with complex government grants.
„« Develop collaborative working relationships with D.A.P. personnel of all levels of authority, community partners, agency consultants, and with city, county, state and federal contract monitors;
„« Delegate and oversee special project work and follow up to assure completeness and correctness of project processes.
