DESCRIPTION

Program Description: BRC's Service Planning and Assistance Network (SPAN) program offers post-release discharge planning services for Brad-H class inmates or those who have been treated for mental health issues, linking them to appropriate continuing treatment upon release from City jails. BRC operates case management offices near the courts in four boroughs and conducts in-reach sessions on Rikers Island three times a week. SPAN is funded by the NYC Department of Health and Mental Hygiene.

DUTIES

Perform outreach efforts to forensic mentally ill individuals released from NYC jails or courts via the courts, attorneys and Department of Corrections. Conduct presentations for service providers and inmates that are incarcerated to inform them of SPAN services. Establish and maintain linkage to mental health, medical and support services. Conduct initial assessments, make referrals to appropriate agencies and formulate discharge plans. Complete entitlements applications. Travel to other boroughs and Riker's Island is required. Provide emergency first aid/CPR assistance when needed. Related duties as assigned.

Full-time, 35 hours per week
Monday-Friday 9:00am-5:00pm

QUALIFICATIONS

BA in social service field (social work, psychology, criminal justice) or 3 years case management experience with clients who have histories of incarceration, mental illness, homelessness and MICA. Computer literacy, oral and written communication skills required. Bilingual (English/Spanish) helpful. CPR training certification or willingness to take training class in CPR.
