Philadelphia FIGHT is a comprehensive AIDS service organization providing state of the art, culturally competent, HIV primary care, and opportunities for clinical research, social services, consumer education, and outreach to people living with HIV and those at high risk. Our goal is to end the AIDS epidemic within the lifetime of people living with HIV now. The Youth Health Empowerment Project (Y-HEP) is one of Philadelphia FIGHT’s Education Programs. Y-HEP works to engage and empower high risk and vulnerable youth ages 13-24 to make healthy choices about their lives through a comprehensive range of services. This position serves the Y-HEP Family Care Clinic, a branch of Philadelphia FIGHT’s federally qualified health center that serves low-income and homeless youth.
Summary : The Eligibility Coordinator is responsible for raising awareness of affordable insurance options and providing eligibility and enrollment assistance to uninsured patients and clients in programs of Philadelphia FIGHT. The Eligibility Coordinator performs their duties within the Y-HEP Community Health Center and may also travel into neighboring communities.
Position Responsibilities/Duties :
Review required financial information with patients verbally and in writing; explain the health center's policy regarding sliding fees and the consequences for not following through with the financial assessment
Collect compliance data for sliding fee documents; follow up and track the patient or responsible party compliance until the necessary documents have been received
Assess patients’ potential eligibility for Medicaid, CHIP and/or Affordable Insurance Coverage, complete Medicaid application if applicable, and enrolls patients in the Affordable Insurance Exchange where applicable
Maintain communication with Social Services to follow up on the status of patient applications when deemed eligible for Medicaid, and obtain written documentation if not deemed eligible for Medicaid
Enter and update collected insurance information in the EMR
Provide information about the full range of Qualified Health Plans options and Medicaid
Assist individuals in applying for coverage through Medicaid and/or qualified health plans. The application process for Medicaid is all year round.
Help facilitate enrollment in Medicaid and/or qualified health plans
Assist patients with prescription navigation to receive a plan for their specific needs and to receive optimal coverage
Agree to serve patient’s best interests, and comply with privacy and security standards
Refer patients to other resources as needed
Perform other duties as assigned Position Requirements :
Bachelor’s Degree Required
Experience working with youth; at-risk youth preferred
Familiarity with and understanding of insurance enrollment and DPW benefits applications.
Candidates must possess strong communication skills, the ability to multi-task and excellent an attention to detail and follow-up skills.
