Hours of Work : Monday-Friday 40 hours per week
Weekend Work : As needed
On-call or Holiday Work :
Education and/or Experience : Associates Degree in Applied Science or Human Services is preferred
One year experience working with a special needs population is required
Valid NYS Drivers license if required
Physical Requirements : Some standing and sitting is required. Ability to handle and lift cases up to 50 pounds. Good mobility is also required.
Job Coach consumers at the job site at the intensity and frequency that is needed by the consumer
Monitor and oversee each consumers production through the provision of weekly production reports
Submit consumers timesheets weekly, ensuring accuracy
Monitor progress and make suggestions for improvement, including referring individuals for the4 next phase of employment.
Instruct consumers regarding all aspects of safety.
E. Complete assigned work tasks along with consumers, to meet production deadlines.
Provide person centered individual support in all areas of supported employment commensurate with the individuals' personal outcomes and goals. Assure that the job placement at the skills, interest and abilities of the consumer.
a. Collect Personal Outcomes information on an annual basis
b. Include the personal outcome information in the ISP
c. Attend ISP meeting for consumers on caseload with MSC services
Coordinate services in the community for each individual as the need dictates.
Make contacts with other service providers in the community to obtain services for consumers, as they are needed
B. Make contacts with ALibert5y staff ( RN,MSC, etc.) as needed to report incidents obtain information or to ensure services are delivered.
Maintain up to date and accurate case records for each consumer on caseload.
a. Scribe progress notes for every service that is delivered.
b. Complete implementation plans and ISP's within 30 days of admission
c. Review implementation plans every six months
d. Complete and submit all OMRDD paper work as outlined in Departmental Policy and procedure.
Maintain good communication in work areas and amongst team members.
a. Attend monthly staff meetings
b. Attend Liberty building meetings
c. Report all incidents to supervisor within 24 hours
d. Attend host company meetings
5. Attend trainings and seminars as required.
