The BHSI ICM unit is seeking to expand capacity and revolutionize under the guide of the DBH/IDS Office of Addiction Services and Community Behavioral Health (CBH) to provide services to all Philadelphia County Residents who are uninsured, underinsured, and those that are health choices eligible. The Enhanced Case Management Specialist, (ECM Specialist) will coordinate formal and informal treatment and provide a strengths-based approach to working with individuals seeking ongoing support needed to facilitate long-term recovery and management of Permanent Support Housing. The ECM Specialist will work in a team model to provide intensive support to those individuals with significant re-occurring substance abuse challenges. The ECM Specialist takes the lead in addiction and recovery and one of five areas of specialty: vocational/employment services, housing, mental health challenges, basic needs/life skills or forensics. Working in a team model, the ECM will support the individual holistically with addressing the challenges to maintaining long-term recovery and identifying, developing, implementing recovery-oriented goals and monitoring the service recovery plan while ensuring that the mission, goals and philosophy of OAS are operationalized within the team.
Duties and Responsibilities:
Provide intensive recovery support services/coordination in a team model to 15-25 individuals with significant and re-occurring substance abuse challenges.
Work independently with 5-10 individuals with lower demand for recovery supports by providing them access to formal and informal recovery supports and by increasing access to community capital.
Must be on-call minimally 7 days per week out of a 30 day period to provide needed intervention and coordination.
Obtain 100% of the available billable units based on the available hours per month
Maintain a quality documentation rating of 95% each month following the documentation rating matrix
Responsible for ensuring the development of measurable unit goals and aggregating outcome data on the area of specialty
Provide workshops to persons in recovery and members of the ICM team around resources and education on the specialized area to improve access
Ensure that the Service Recovery Plan reflects measurable goals and action-steps that reflect specialized domains
Monitor the quality of treatment through social services agencies, residential and outpatient treatment facilities
Participate in the individuals treatment experience by closely working with the recovering persons care manager, therapist, psychiatrist, social worker, etc.
Coordinate clinical interventions as needed
Attend all mandatory trainings as designated
Complete required paperwork and documentation timely and according to agency standards
Perform other duties as assigned
Must have excellent verbal/ written communication skills, be detail oriented with exceptional time management and organizational skills and the ability to work well with individuals experiencing significant substance abuse challenges. Critical decision-making skills and the ability to work independently and within a team is a must. Must be proficient in using Microsoft Office, databases, web-based browsers and basic typing.
