Purpose: To provide support and assistance with system navigation to families in any of Cuyahoga Tapestry System of Care initiatives (CTSOC) in their own or assigned neighborhoods.
Ensures that Family voice is present at all Welcome meetings to explain the role of the Family Advocate to families.
Actively participates in all scheduled Child and Family team meetings for the assigned family.
Immediately notify Care Coordinators and Lead Advocates in the event of any unusual incident.
Attends all transition meetings to inform the families of opportunities available in their community.
Conducts home visits to complete tasks assigned by the family team.
Identifies traditional and non-traditional services from community and collaborative sources needed to meet the needs of the children identified during the family team meetings.
Facilitates monthly support groups and encourages family attendance.
Tracks parent contacts and submits documentation and reports as required.
Distribution and tracking of welcome and transition packets.
Attends required CTSOC trainings, learning communities, and other meetings as determined by your agency and the ASO.
Be knowledgeable regarding behavioral health and the child serving systems.
The ability to empathize with children and adults who are experiencing crisis situations, from a wide variety of cultural backgrounds and socio-economic status.
Have good people and communication skills.
Be committed to the purpose and goals of CTSOC.
Help parents of the children and youth enrolled in the CTSOC initiatives acquire and strengthen positive behavior management skills.
Be able to work a flexible schedule in your community.
Participation in Family Advocacy Training and Skills Development.
Bachelor's Degree in Social Work or related field.
Parent or Caregiver of a youth who has received services from child serving agency.
