Family Care Advocate (FCA) will become part of the Pediatric Center for Comprehensive Care and work with a physician extension care team consisting of physicians, nurse practitioners, nurses, social workers and nutritionists to provide comprehensive, coordinated care for children with complex, chronic conditions.
Specific responsibilities include assisting families and patients in coordinating visits with providers, tests and services within the hospital and community. Accompany patients and families to provider visits to facilitate communication and assure adherence to care plans. Identify stressors both medically and psychologically and communicate them to the team. Administer validated assessments to measure family functioning and quality of care. Establish a relationship with patients and families to reduce stress and improve function. Perform home visits to identify care plan barriers and create solutions to overcome barriers.
Position Requirements: Minimum 2 year experience as a Medical Assistant Demonstrated skill in interpersonal relationships Highly motivated individual who is able to function independently and with teams Must have excellent organization and time management skills. Computer, fax machine, copier, Microsoft Word and Excel Ability to multi-task in a variable speed environment Ability to set priorities and work effectively under competing deadlines and changing workloads. Must have flexibility in job hours to adjust to sudden changes in workload/job requirements.
Education Requirements: Successful completion of a Medical Assisting certified program and certification in Ohio preferred equivalent pediatric experience Experience working with children and families with chronic health conditions preferred. Experience providing guidance and education to parent/families, medical community and general public preferred.
License Requirements: Certified Medical Assistant; Drivers license and access to a vehicle
