Community Counseling & Mediation (CCM)'s Family & Treatment Rehabilitation Program is designed to address the safety and well-being of children in high-risk families when a caretaker or child's substance use or mental health disorder places the child at risk of foster care placement or replacement.
The Supervisor will be expected to carry a small caseload, and may accompany case planners on some home visits.
CCM seeks a Supervisor who will hold an LCSW. Duties will include but are not limited to:
-Conduct assessments including: family and developmental history especially related to substance use and mental health disorders, family interaction and current family functioning.
-Work collaboratively with the AC's Borough Office and other referral sources to identify the needs of the children and caretakers in order to coordinate and develop plans for new and on-going services.
-Provide individual, family and/or group therapy.
-Coordinate services and participate in CDT meetings.
-Participate in/facilitate Family Team Conferences (FTC).
-Provide weekly supervision to approximately 5 case planners.
-2 years experience in a supervisory role in a social service or clinical setting
-An ability and willingness to work with families within the home.
