Ensures all authorized home care services needed to safely maintain clients in the community are provided in a timely manner. Expedites all necessary action for compliance of the provider to the established service plan. Identifies and advocates for other services to assist the family meet short and long term goals, and liaisons with City and other involved agencies towards maintaining a current home care authorization to ensure that services are coordinated.

Knowledge, Skills And Requirements

Must have a Bachelor's degree and a minimum of one year experience providing direct services to individuals and/or families in need. Experience could be in a variety of health, mental health, child welfare, protective, and preventive and/or community services. Preferences will be given to candidates with experience in the home care field. A Masters Degree in Social Work or a related field may be substituted for experience.
