Description:
Works in collaboration with RN Transitional Coordinator of Project RED in transitioning patients to post discharge care. Pharmacist reviews and reconciles home medications with patients upon admission. Provides patient education regarding medications on an ongoing basis while patient hospitalized, reviews discharge medication plan with physicians to ensure accuracy, and reviews home medication regimen with patient at the time of discharge. Conducts post discharge phone folow ups calls with RN Transitional Coordinator to address any medication educational needs after discharge.
Requirements:
Completion of course at an accredited School of Pharmacy, with a Bachelor of Science Degree in Pharmacy or Doctorate in Pharmacy.
NJ license as a Registered Pharmacist
Hospital Pharmacy experience preferred
Familiarty with all phases of Hospital Pharmacy required
Working knowledge of Health Care Matters
Understanding of role of Pharmacy Department with the Acute Care setting, its interrelationships with other departments and functions
Pharmacy Computer applications required
Competency for IV Admixture preparation including Chemotherapy
