LOCATION: 300 S. McCormick St, Prescott, AZ
The Arizona Pioneers' Home is a continuing care retirement home operated and funded by the State of Arizona. The Home provides care and services to residents living independently, as well as to those requiring personal, assisted living, intermediate and skilled care; qualifications for admission are defined in state statute. Pioneers' Home is currently seeking a HUMAN SERVICES UNIT SUPERVISOR to join the team.
POSITION DESCRIPTION-This position is responsible for providing social services to residents of the Arizona Pioneers' Home.
RESPONSIBILITIES INCLUDE-Regulatory compliance; administrative functions, QA/QI; assessments and care planning; resident advocacy; assist residents with personal needs; advance directive planning and records management pertaining to social services. Participates in admissions and marketing. Annual review and selection assistance of residents' Medicare D coverage. 10%-30% regional, daytime travel.
KNOWLDEGE SKILLS AND ABILITIES-The individual selected to this position must have considerable knowledge and experience of long term care social services programs; State and Federal laws and regulations governing long term care and/or assisted living; State and community services on a local and State wide basis, knowledge of Medicare and Medicaid; ability to read, write and speak the English language and communicate same with excellent communication skills; resident advocacy and care-planning. The individual selected will be able to interact well with persons of varying cultural, socio-economic, cognitive and educational levels. Ability to work within a team concept a must.
THE IDEAL APPLICANT will have a Bachelor's degree in social services or related field and two years recent experience in long-term care or assisted living in the state of Arizona.
A medical/physical evaluation is required prior to appointment.
