Maintenance of all inpatient records by assigning ICD-9-CM and/or ICD-10-CM/PCS according to published coding guidelines.
Principal Duties and Responsibilities :
Accurately codes inpatient discharges using ICD-9-CM and/or ICD-10-CM/PCS coding system .
Assembles medical records in defined order.
Retrieve all charts from nursing units upon patient’s discharge.
Processing of written, phone, and walk-in requests for release of medical information.
Files loose inpatient reports.
Retrieval of charts for committees and chart review requests.
Answer phones and assists in other department coverage.
Coordinates review activities for the “Ongoing Record Review” and collates results for reporting to the Record Review Committee.
May perform transcription duties.

JOB REQUIREMENTS & CHARACTERISTICS
Skills & Abilities
Medicare/DRG skills, complete medical records skills including confidentiality/release of information and basic clerical skills.
Knowledge & Experience
Must have knowledge of ICD-9 and CPT4 coding.
Previous medical record experience preferred, but not required. Ability to transcribe radiology and basic 4 .
Certified Coding Assistant.
