Health Information Technician is a clerical position responsible for ensuring medical record documentation is completed accurately.Completes chart compliance, record review and identifies documentation issues to assure documentation is completed in a timely manner. Follows up with caregivers for completion of mandatory forms and needed documentation to ensure timely collection of documentation and to comply with the State and Community Health Accreditation Program regulations. Responsible for electronic submission of Outcome and Assessment Information Set to Centers for Medicare and Medicaid per facility policy and procedure. Responsible for tracking, faxing and follow up on physician orders and certifications/plan of care. Ensures physician signatures are obtained in a timely manner not to exceed the Agency policy.

High school diploma or equivalent. One to three years experience in a Health Information Management department, with experience in electronic records preferred.
