Summary of Job Duties
The Home Health Nurse Liaison is a licensed professional who collaborates with patient, family, physicians, case managers, health care team, and community resources to develop a plan which addresses the patient’s post-hospital medical needs. They assist and provide patient care coordination and referral services for patients discharged from an in-patient setting or other outside sources and refer them to the home health agency.
Qualifications & Requirements
Ability to communicate effectively (verbally and written)
Computer knowledge with ability to utilize software based on organization standards and requirements
Current California RN license
Experience in sales and marketing preferred
Graduate from accredited school of Nursing
Knowledge of Medicare, Medi-Cal and community resources
Knowledge of services provided by all levels of home care services
Maintain a clean driving record with a valid California driver’s license
Minimum of 1 year home health experience within past 5 years
Minimum of 2 years clinical experience as a registered nurse
Must possess Current American Heart Association BLS or American Red Cross Professional Rescuer Certification
One-year experience as Discharge Planner preferred
