Position Summary
This position exists to coordinate pre-hospice patient/family education, schedule admission RN visits, and provide coordination of services at start of care, data collection and informed consent activities. This person will work closely with the interdisciplinary team to determine the patient/family needs, schedule appropriate RN, order equipment and coordinate communication to assure optimal delivery of care. Is responsible to review schedule and communicate and scheduling needs to manager or director. Minimum Qualifications
Education, Credentials, Licenses: High school diploma: college-level education preferred
Specialized Knowledge: Hospice benefit and OhioHealth Hospice program knowledge. Knowledge and profiency of scheduling. Previous office or intake experience. Excellent customer service skills. Ability to talk to patients, families and other community representatives with confident public speaking skills. Awareness of cost management and resource utilization.
Proficient in medical terminology
Kind and Length of Experience: 3-5 years in office management, sales, marketing or communications. Additional Job Information
This admission coordinator position works closely with the hospice admission team. Daily interaction with referral sources, families, and loved ones. This is a fast paced working environment providing access to patients and families that need hospice services. Potential to rotate to Saturdays as needed. Scheduled Hours Per Week
