The Hospice Aide provides personal care, respite, companionship, light housekeeping and assistance with meals for patients. Participates in care planning as a part of the interdisciplinary team. Renders or assists with personal care/ADLS such as: bathing, change of bed linen, shampooing, shaving, mouth care, etc. Assist with ambulation, transfers, and/or normal range of motion and positioning. Obtains patient vital signs and record on appropriate form if assigned to do so. Reports any changes in patient/client condition and needs to the Registered Nurse or appropriate supervisor. Maintains a clean, safe, healthy environment. Observes for adequate nutrition/fluid intake. Observes basic infection control techniques. Recognizes emergencies and acknowledges emergency procedures. Completes progress notes on all patient/client visits on care furnished.
High School education or GED required. Current certification in the State of Michigan as a Certified Nursing Assistant preferred, Home Health Aide certification will be considered. Valid Driver’s License required. Previous experience in long term care nursing home, hospital, hospice or home care agency preferred. Ability to work extended hours beyond normal work schedule to include, but not limited to, evenings, weekends, extended shifts and/or extra shifts, sometimes on short notice.
