Steward Home Care is seeking Hospice Nurse Practitioners with current clinical experiences that are enthusiastic, energetic and compassionate to be part of an Interdisciplinary team providing Hospice Care in the North Eastern and Central and South Eastern MA Communities we serve.
Responsibilities include:
The Hospice/Palliative Care Nurse Practitioner (NP) will work as a member of the interdisciplinary team under the direct supervision of the Medical Director
The NP will work in collaboration with the hospice Medical Director and is directly accountable for making clinical judgments using independent and interdependent decision.
The NP will perform patient care such as symptom management during visits in the hospital and community setting.
The NP is responsible for identifying patient/family needs and providing supportive care and appropriate services, in accordance with the primary care physician’s orders and the plan of care.
The ideal candidate will be able demonstrate skill in the practical nursing practice, should be knowledgeable in measures to reduce pain and promote comfort, be able to think on their feet and have the ability to cope with family emotional stress.
Must be a board certified Nurse Practitioner. HPCN certification and completion of ELNEC training preferred. Previous hospice experience in a hospice / home care environment is preferred along with an understanding of the principals of pain/ symptom management. Palliative care experience strongly preferred.
