Under the supervision of manager, provides advanced and comprehensive case management consultation for families with children who have health risks, conditions or special health care needs. Oversees case management services for families in the Children with Special Health Care Needs (CSHCN), Supplemental Security Income (SSI), Newborn Screening (NBS), Personal Care Services (PCS), Children's Health Insurance Program (CHIP), Child Protective Services (CPS)/Foster Care and Case Management for Children and Pregnant Women programs. Assists families with the eligibility determination process for CSHCN-SP. Travels to conduct home visits. Completes Intakes, Family Needs Assessments, Service Plans, and Follow-up Services to families on caseload. Links families to community resources. Conducts comprehensive assessments to determine eligibility and need for PCS/PDN. Conducts presentations and communicates program services and/or changes to consumers/public in a positive and accurate manner. Works as a team member. Actively participates and advocates in Community Resource Coordination Groups (CRCG), case management coalitions, and community organizations. Provides information and referral services. Completes administrative duties, monthly reports, special assignments and work activities in a professional and timely manner. Works collaboratively with public health staff in the event of a regional or state disaster. Works under minimum supervision with considerable latitude for the use of initiative and independent judgment. Maintains confidentiality.
Essential Job Functions:
Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned. (50%) Case Management Functions--Provides complex and specialized social work consultation and comprehensive case management services to families who receive services or are seeking services from the CSHCN-SP, SSI, NBS, NBHS, CLPPP, CHIP, CPS/Foster Care and Case Management for Children and Pregnant Women programs. Conducts home visits for comprehensive assessment of home environment and needs. Following program guidelines, provides comprehensive case management services including the Intake, Family Needs Assessment, Service Plan, and Follow-up services. Assists families with the eligibility determination process for CSHCN-SP. Provides information and referral services, linkage to community resources, family supports, advocacy, and service coordination. Maintains documentation of work activities with record documentation, monthly reports, travel reports, case summaries and correspondence. Utilizes software programs to do database entry, word processing, spreadsheets, electronic mail, and Internet. (30%) Conducts comprehensive assessments to determine eligibility and need for PCS/PDN. Conducts assessments during home visits with families. Documents identified needs and forwards documentation for authorization of personal care services. Conducts reassessments for PCS/PDN as required or as necessary according to family needs. Enters family encounter information into an online database. (5%) Technical Assistance/Quality Assurance/Trainings – Provides consultation, presentations, and training to service providers/contractors for programs such as CSHCN-SP, Case Management for Children and Pregnant Women and Family Support Services. Participates as a team member to conduct monitoring site visits to evaluate and provide technical assistance to providers/contractors/potential providers on case management issues. Performs public relations with providers and recruits new providers. Works as a team member to develop improved processes and trainings for providers, contractors and consumers. Assists in training new staff, volunteers and social work interns. Provides program presentations to public to promote awareness of services available to children with health risks, conditions or special health care needs. Participates as a team member with peer chart reviews for regional internal quality assurance. (5%) Community Organization/Resource Development -- Actively participates in community organization functions and collaborates with other community entities on health, social work, case management and other related issues. Serves as the representative for the Texas Department of State Health Services (DSHS) on the CRCG, coalitions, interagency groups and advocacy groups to collaborate efforts in serving children with special health care needs. Participates in the delivery of community disaster mental health services by collaborating with groups such as the American Red Cross. Maintains social service directory of community resources for service area. (5%) Professional & Team Development -- Attends trainings and conferences relevant to job to meet licensing requirement and provides backup and support to other team members. Continues to develop professional knowledge and skill in the practice of Social Work/Case Management in the Public Health setting. (5%) Marginal Functions--Participates in performance evaluations process. Attends staff meetings. Performs other duties as assigned. In the event of a regional or state wide disaster, all staff will participate, assist, and support all regional and/or state wide disaster relief efforts. Responds to and supports regional/statewide disaster relief efforts.
Knowledge Skills Abilities:
Knowledge of Case Management principles and practices. Knowledge of children's programs, such as Medicaid, CSHCN, Case Mangement for Children & Pregnant Women, CPS/Foster Care, SSI, NBS, NBHS, Lead, Early Childhood Intervention (ECI), and CHIP. Registration or Licensure Requirements:
Current permanent license to practice as social worker in Texas (LBSW, LMSW, LMSW-AP, or LCSW) *Continuation of Job description: Staff required on an as-needed basis to assist with a response and/or recovery effort because of their subject matter expertise and/or experience.
