POSITION OVERVIEW:

The IICAPS Mental Health Counselor provides services to families in their homes and within their time schedule to eliminate barriers to treatment. Collaborate with clinician in assessment and treatment planning. Offer therapeutic support, parent guidance to families. Work individually with children as clinically indicated. Collaborate with clinician to provide advocacy, case management and concrete services in accordance with treatment plan.

ESSENTIAL QUALIFICATIONS/SKILLS:

Collaborate with clinician in assessment and treatment planning.
· Offer therapeutic support, parent guidance to families.
· Carry a case load of 8 to 9 cases; averaging 5.5 hours per seek per case
· Work individually with children as clinically indicated.
· Collaborate with clinician to provide advocacy, case management and concrete services in accordance with treatment plan.
· Must be willing to schedule family visits during late afternoon and evening times and/or weekend hours
· Attend IICAPS trainings and adhere to Yale IICAPS model as overseen and directed by Manager
· Provide after hours on call as part of regular rotation (may be required to go into home)
  • Valid driver’s license and good driving record.
      PREFERRED QUALIFICATIONS:

      Bi-lingual (Spanish) desired.
