DESCRIPTION

The Intake Specialist is responsible for processing referrals, file establishment and maintenance, client intake and orientation and general process management to assist with the provision of services.

DUTIES

IISPECIFIC RESPONSIBILITIES
Specific Responsibilities for the Intake Specialist may include, but are not limited to:

1.Receive, review, and summarize referrals for all day services; community employment services and respite services. Maintain and review processing list for all day services locations.

2.Ongoing communication with funding source.

3.Prepare and route as necessary Reference Review Form (208) for Admission Team.

4.Establish and maintain Master File.

5.Schedule & facilitate Intake and Orientation, prepare and complete paperwork.

6.Coordinate and conduct conferences needed.

7.Maintain documentation as required by CARF, funding source, and agency.

8.Provide referrals to other resources as needed.

9.Schedule tours for perspective participants, families, providers, Case Managers & outside professionals.

10.Participate in meetings, conferences and workshops as required.

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
1)While performing the duties of this job, the employee may be required to climb ladders/stairs, walk, sit, stand, stoop, bend, squat, push and pull.
2)Without assistance, employee may be required to lift and/or move up to 10 pounds.

3.General Tasks and Abilities
1)This position requires the ability to operate a vehicle
2)Employee must have the ability to read and comprehend documents such as safety rules, procedure manuals, professional publications and operating and maintenance instructions.
3)Employee must have the ability to write routine reports and correspondence.
4)Employee must have the ability to comprehend, retain and follow oral and written instructions, exercise good judgment, set priorities, concentrate and focus on sensitive materials or items, listen, analyze, problem solve and make decisions.
5)Employee must have computer skills regarding data entry and be proficient in Microsoft Windows, Word, and Excel.
6)Employee must be able to make informative and persuasive presentations, negotiate, self-direct, work without supervision, function in high stress situations, provide instruction/training, direct, inspect and organize.

4.The Intake Specialist shall assume other responsibilities as requested by the Social Services Senior Director, JobSource Director, Supported Living Director, or Respite Services Manager.

QUALIFICATIONS

IVQUALIFICATIONS
To perform this job successfully, an individual must be able to perform each of the Specific and General Responsibilities satisfactorily. Additionally, the requirements listed below are representative of the education and/or experience required to perform this job:

1.Required Licenses, Certificates, Degrees and/or experience
1)Must possess a valid Driver's License, with a driving record acceptable to Opportunity Enterprises, Inc. insurance carrier.
2)Must have a High School Diploma, or GED equivalent.
3)Must have 4 years experience in the Developmentally Disabled Field.
4)A Degree in Social Services is preferred.

2.Continuing Requirements
Must keep all certificates, licenses, and registrations current.
