Coordinates services for individuals eligible for the Independent Care Coordination Program (ICCP). Processes and documents Care Coordination Supplemental Funds. Develops and fosters personal and community supports within the service area. Facilitates admissions and/or discharges to/from State Hospital or Diversion Facility. Coordinates ICCP services with other Telamon programs as indicated. Adheres to corporate, state/federal agency and private funding reporting requirements by maintaining data measures and documentation according to contract, grant and/or corporate guidelines.

QUALIFICATIONS:
Bachelor degree or evidence of certification or licensure in social work or a related field required. Two years experience in working with mental illness required. Experience working with substance abuse preferred. Experience with Microsoft Office or comparable software required. Must have effective oral and written communication skills, and the ability to work with individuals at various socio-economic levels. Bilingual in English/Spanish preferred.

PHYSICAL REQUIREMENTS:
CERTIFICATES, LICENSES, REGISTRATIONS: Attends and completes training required by the BHHF “Standards for Care Coordination Training” within 1 year of hire date, to include: Assertive Engagement/Motivational Intervention Techniques, Wellness Recovery Action Planning (WRAP), Illness Management Recovery (IMR), and Positive Behavioral Support (PBS).

The successful applicant must have: Ability to use personal computer. Ability to sit for extended periods. Ability to work under stress and/or emergencies situations. Extensive travel required; must possess valid driver’s license and reliable transportation.

WORK ENVIRONMENT: Standard office environment and field work. Business dress according to office policy.
