The purpose of this position is to evaluate and establish the appropriateness of Home Care Referrals and to obtain authorization for care as needed.
Nature and Scope:
The Home Care Intake Coordinator will review and analyze all Home Care referrals for appropriateness, ability to accept and prioritization of need. The clinician will be responsible to gather all pertinent information and to confirm information provided by referring entity. The clinician will review health records to identify relevant diagnoses and procedures pertinent to Home Care. The clinician will obtain physician orders as necessary for home Care staff to provide skilled care. The Intake Coordinator will also be responsible for obtaining insurance authorization for specific payers.
The clinician must have a thorough understanding of the content of the medical record in order to be able to locate information to support the need for home care. The clinician must be trained in the anatomy and physiology of the human body and disease processes in order to understand the etiology, pathology and symptoms needed to be treated in the home care setting.
This position is challenged to be aware of the continual changes in Federal and State regulations for prospective payment, keep informed of changes in treatment modes and new procedures.
This position may provide patient care.

Minimum Qualifications: Requirements - Required and/or Preferred
Must have working-level knowledge of the English language, including reading, writing and speaking English.
Minimum two years acute hospital experience required. At least one year of experience in Home Care preferred.
Ability to obtain and maintain a State of Nevada Licensed Practical Nurse license. Valid State of Nevada or California driver’s license and ability to pass Renown Health’s Department of Motor Vehicles Report criteria and current automobile insurance.
Current CPR/BLS certification by AHA standards required.
Computer / Typing:
Working knowledge of basic computer programs and Microsoft Office Suite. Keyboard typing required. Must be proficient with Microsoft Office Suite, including Outlook, PowerPoint, Excel and Word and have the ability to use the computer to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc.
