The Intake Specialist is responsible for the receipt/input/review/completion of all referrals. Does initial assignment of evaluation visits. Contacts patient/family to notify of services that will be provided. Facilitates the Face 2 Face Process and owns follow through until completion. Works with Centralized Insurance Team.
The Intake Specialist reports directly to the Director of Operations.

Intermediate computer skills. Proficient in Microsoft Office and Windows.

Minimum Educational/Experience Requirements:
Minimum of 2 years Medicare home care experience.
Expert working knowledge of Medicare covered home health care and other insurance programs, regulations and guidelines regarding home health care.
Knowledge of Federal and State regulations.
Maintain current health requirements.
