MBHP, a leader in behavioral healthcare in Massachusetts is seeking a licensed clinician, registered nurse, or clinical psychologist to join our clinical team of Integrated Care Managers to support a new, expanded contract.
The Integrated Care Manager is responsible for the clinical management of all assigned Care Management Program Members who meet enrollment criteria, focusing on members who are identified as high risk with a behavioral and medical co-morbidity. Knowledge of the treatment and coordination of care for Members who are experiencing complex mental health, medical, and/or or substance use conditions is required as an understanding of the basic concepts of care management and managed behavioral health care. This position also requires a valid driver’s license as requirements will include travel for frequent member contact and community outreach.
Responsible for evaluation, determination and authorization of appropriate levels of care based on established criteria for MassHealth and state agency consumers.
Responsible for initiating contact with a designated minimum number of assigned members; explaining the care management program to them and enrolling them in the care management program. This may involve phone contact, written contact and or face to face contact in the community in order to reach members.
Once members have been enrolled conduct planning, convening and leading treatment planning/care coordination meetings in community with members. Utilize individual Member medical, behavioral and utilization data to co-create, with Members and ESPs, individual crisis plans and coordinate their care by reviewing clinical information, on-site and telephonically, regarding requests for authorization for payment of care.
Implements the comprehensive plan of care. Build relationships and assists members and families in understanding and carrying out treatment care plans with a primary emphasis on meeting members face to face and in the community. These activities will involve frequent member contact and outreach in the community. In the course of their work employees may be required to transport members as clinically necessary.
Recommend and arrange discharge plans, aftercare referrals, and community resources when members have an inpatient psychiatric admission. Consult with providers to assist Members with transportation and employment issues and daily living needs as they relate to individualized care plans.
Education: Master’s level
Licensure: licensed mental health clinician as an RN, LICSW, LMHC, LPC, PhD, Clinical Psychologist or LMFT
Relevant Work Experience: 5 years work experience in a mental health setting. 2 must be post-masters. Knowledge of treatment care resources as well as available levels of care, ability to relate effectively with behavioral health and medical treatment providers, including MBHP Members, family members and other professionals. Must be authorized to drive and have a valid license.
