General Statement of Duties: Ensure that all families referred to the program receive intensive flexible family supports reflective of their unique needs. Work cooperatively with the Brockton Area DDS office and function as a team member in the Family Support Program.
Facilitate the development of Family Support Plans that are based on the strengths and needs of the family, identify
resources, and plans to access those resources including specific assignments and timelines. These plans should include generic resources, community based services and any specialized services; and will take into account a familys particular circumstances.
Ensure the development of appropriate responses to behavioral challenges presented by the person who is the focus of the service. Make appropriate referrals to Behavioral Specialists and work
closely with the family and the specialist in the implementation of any Behavior Plans. Maintain a liaison relationship with the DDS, BAMSI and other specialists as appropriate.
Develop and maintain relationships with local School Department staff and ensure continuity in responses to behavioral challenges
Ensure the development of an emergency/crisis plan for each family as appropriate.
Link referred families to community services such as
health resources, economic resources, mental health resources, educational resources, etc. Many of these services may be provided by BAMSI and if so, facilitate smooth access to these services by maintaining relationships within the organization.
Identify, develop and maintain a comprehensive network of resources, services and supports that are available within the community and refer families to same as appropriate.
Work collaboratively with the Brockton Department of Developmental Services staff.
Maintain appropriate documentation. Maintain a data collection system consistent with funding source requirements.
Manage flexible funds allocated to families as appropriate.
Perform other related duties as assigned.
